Attacks in Iraq on U.S.-led forces, local security personnel and civilians in the three months to Nov. 10 have surged 22 percent over the previous three months to reach an all-time high, the Pentagon said in its latest quarterly report on Iraq published Monday. The report also noted a rise in civilian casualties and said this was directly linked to the rise of sectarian death squads, which received help from elements of Iraqi forces. Full Story
