Officials say water scare could have been averted

Phoenix officials admitted Tuesday that January’s water contamination scare could have been avoided and that the incident exposed serious vulnerabilities within the city’s emergency response network. A preliminary city audit has also concluded that there were breakdowns in communications and procedure during the days and hours leading to the Water Services Department’s Jan. 25 decision to issue a boil-water advisory to the city’s 1.5 million residents.
